Abstract: Ventilator Associated Pneumonia (VAP) is one of the most common device related infections in ICUs and although with advanced intervention strategies yet it is associated with high mortality rate. Prevention of VAP is vital while caring for mechanically ventilated patient. The nurse's role in prevention of VAP is extremely significant as they are the primary care givers and provide constant care to the critically ill patient. The research was conducted to evaluate the impact of structured teaching program on ICU nurses regarding prevention of ventilator associated pneumonia. The main objectives of the research study were to assess ICU nurses knowledge about prevention of VAP, to evaluate the effectiveness of structured teaching programme on prevention of VAP among ICU nurses and to determine the association among ICU nurses knowledge regarding prevention of VAP with their selected demographic variable. Method: An evaluative research model was used without any control group using pre- experimental design, with purposive sampling technique with one group pre-test and post-test. 60 Registered ICU Nurses were participated in study and evaluated using semi- structured knowledge questionnaire. STP was implemented, using same tool 7 days later post-test was conducted. Results: Overall, pretest knowledge scores on prevention of VAP was found to be 49.8% and the overall posttest knowledge scores were recorded as 74.4% thereby 24.6% of enhancement in the mean percentage knowledge score was found which is significant at with P<0.001 level. Also significant association between knowledge scores & selected demographic variables were found with age (χ2 28.01), marital status (χ2 37.51), income (χ2 36.11), professional educational qualification (χ2 24.75), clinical experience (χ2 37.98), knowledge about VAP (χ2 38.5) and source of information on VAP (χ2 16.93) and no association found with gender. Conclusion: The study concluded that knowledge deficiency persist among staff nurses regarding prevention of Ventilator Associated Pneumonia and STP shows efficiency in improving their knowledge.

Abstract: Post stroke care is ever evolving from the acute in-hospital stroke unit approach to the long-term stroke rehabilitation process that extends beyond the hospital settings. This is in accordance with the recent development in the neuroplasticity process which now believed to continue beyond the six months sub-acute recovery phase. A long-term comprehensive stroke care beyond in-hospital treatment through a multidisciplinary approach has been practiced in developed nations and proven to have favorable outcome for post stroke patients. This has lessened the stress in the medical system which has become increasing exhausted due to the increasing prevalence of stroke cases and the over burden in handing the COVID-19 pandemic. The lockdowns during the COVID-19 pandemic have disrupted the usual stroke care in countries with an already scarce resource like Malaysia. Therefore, a more practical approach in utilizing resources available in the community can be a solution to optimize stroke care in the community. In this review, we shared our experience in managing post stroke patients using a comprehensive primary care team approach in providing a holistic multi component post stroke management in the community settings. The model integrates various disciplines that combine preventive and rehabilitation treatment for post stroke patients in a coordinated systematic approach.

Abstract: Misuse of anabolic-androgenic steroids (AAS) is on the rise globally, particularly among male gymgoers who use them to gain muscle and enhance their physical appearance. Because AAS abuse affects the hypothalamic-pituitary-gonadal axis and can temporarily or permanently reduce spermatogenesis, it is thought to be a preventive factor in male fertility. The study aimed to examine the impact of anabolic steroid abuse based on semen characteristics, serum FSH, LH, and free Testosterone horhormonevels suggesting a reduction in male bodybuilders' fertility. Eighty healthy male gym-goers from Mosul city were separated into the case and control groups for a prpective cohort research. For a period ranging from September 28, 2020, to March 15, 2021, a structured interviewing questionnaire was employed to gather data on AAS misuse, blood sampling, and semen specimen for examination of serum levels of FSH, LH, Free T, and sperm parameters, respectively. The AAS group experienced a significant drop in the mean serum levels of luteinizing hormone, follicle-stimulating hormone, and free testosterone (P = 0.000). According to a semen study, the overall mean of the total count of sperms per ejaculate was (18.83 ± 24.02 vs. 139.5 ± 24.80) As compared to the control group, the proportion of sperm motility was significantly lower. The risk of developing oligospermia is five times higher in the AAS group compared to the control group (RR: 5; 96 percent CI: 2.690-9.293; P => 0.001). Male fertility is significantly impacted by the use of AAS; users showed significantly reduced levels of gonadotropins, which may indicate hypogonadism, as well as low free testosterone and sperm counts, both of which may indicate poor spermatogenesis. The study advises against anabolic steroid addiction and warns against its negative side effects, which have a particularly negative impact on fertility.

Abstract: Systemic hypertension is a major risk factor for cardiovascular diseases, and it is the most important but modifiable risk factor for all-cause morbidity and mortality worldwide. It is considered a complex and polygenic medical condition and influenced by genetic, environmental, and social determinants including unhealthy diet and lack of physical activity. Despite the available evidence strongly supporting the positive impact of physical activity in the prevention and treatment of hypertension in primary prevention, challenges remain to the medical and health care communities to incorporate this information into the daily practice of medicine. The purpose of this review is to investigate the effect of physical activity on systemic hypertension by presenting an update of the literature to strengthen current recommendations. Several search engines were reviewed, including PubMed, Medline, Cochrane Library, and Google Scholar using key words, hypertension, and physical activity. Populations under the age of eighteen were excluded from all nineteen reviewed studies. All the reviewed studies concluded that physical activity reduces blood pressure in men and women eighteen years of age and older. Physical activity is a recommended non-pharmacologic tool to treat hypertension. Currently, more than 75% of the US adult population does not meet the recommended physical activity guidelines. Consequently, primary care providers may collaborate with other professional health instructors to educate their patients about the current guidelines and recommendations regarding physical activity, and further discuss the effect of physical activity on hypertension.
